What hasn't happened yet but might still happen if the Russian military continues to fall apart, is that the Russians might go back to their WWII practice and simply execute anybody who retreats or deserts. When you see that the Russian soldiers are departing and letting the Ukrainians capture (and use) their tanks and other equipment, a very heavy response is going to be the only way to change their behavior.

I have brought that up too along with sending men into combat with no weapons like was done in the past.

In WW2, Russia suffered from massive casualties by using the above tactics! But, they counted on the massive population of eligible men to replace them and attacked the German lines by over powering them! Germany did not have the men needed to do that.
